Teens Taking Charge: Hemophilia

    The Challenge
    Teens living with hemophilia will eventually have to switch from a pediatric clinic to an adult clinic. This transition impacts both teens and their families. Not only will teens have to adjust to a new care team in a new location, but they will also need to learn to be more independent, confident, and responsible in their own hemophilia care. This transition can be a stressful period for teens and their families.

    The Goal
    Together with AboutKidsHealth, the experts at SickKids aim to create a comprehensive program that helps to ease teens’ transition from pediatric to adult care.

    The Solution
    The Teens Taking Charge: Hemophilia program breaks down the key information into 8 modules that cover everything from the basics and complications of hemophilia to looking ahead beyond the transition of care. The program uses teen-friendly visual styles to present the information in text, animation, interactive module, and video formats.
